Advanced breast cancer was killing her. A radical new therapy saved her

The decades' long study found that women with the most common types of breast cancer can avoid chemotherapy.

On Sunday, within hours of the trial results being presented at American Society of Clinical Oncology (ASCO) 2018 meeting in Chicago, the 48-year-old emailed them to her doctor with this line: "This is good news for me".

"It feels miraculous, and I am beyond amazed that I have now been free of cancer for two years", Perkins told The Guardian.

The study, published June 3 in the New England Journal of Medicine, finds that most patients with an intermediate risk of cancer recurrence can skip chemotherapy treatment without affecting their chances of surviving the disease.

While the USA doctors who developed the therapy can not be sure how much the infused immune cells contributed to her recovery, the use of pembrolizumab alone has not been very effective for advanced breast cancer in the past. Well, a groundbreaking new study says that doesn't need to be the case. An estimated 3,000 to 5,000 of women in the United Kingdom are likely to avoid chemotherapy every year as a result. Twenty-five percent of those patients won't qualify for chemotherapy because of age or medical problems.

In the trial, researchers gene tested more than 10,000 patients across six countries with the most common form of early breast cancer, hormone-receptor positive, HER-2 negative breast cancer that hadn't spread to the lymph nodes. Oncotype DX costs around $4,000, which Medicare and many insurers cover. However, there was a range between favourable and unfavourable where it was not clear whether we should give chemotherapy. "[The findings] are both important and significant, and also practice-changing", says, Dr. José Baselga, a medical oncologist and physician in chief at Memorial Sloan Kettering Cancer Center in NY, who was not involved with this research. After an average of nine years, 83.3% of those who did not undergo chemotherapy remained cancer-free, compared to 84.3% of those given chemo.

It works by looking at the activity levels of 21 genes, which are markers of how aggressive the cancer is.

About 17 percent of the women tested had high-risk scores and were advised to have chemotherapy. The results are similar for women under 50 with a score between 0 and 15. We owe those who took part in this trial our thanks.

The group is getting better at identifying the rare T-cells that can attack the tumor, finding three times more of them in samples than they were even three months ago, Rosenberg said.

Professor Arnie Purushotham, senior clinical advisor at Cancer Research UK, said that by grouping patients based on how likely their cancer is to return, the trial shows great potential to ensure treatment is kinder without compromising its effectiveness.

After enrolling for new trial in 2015, doctors in the U.S. adopted an experimental approach combining two different forms of immunotherapy after conventional hormone treatments and chemotherapy failed.

Researchers at the National Cancer Institute have put a patient with metastatic breast cancer into complete remission using a new approach to immunotherapy.
